You must submit a completed University consent form and/or consent recording when submitting:
- a clinical assignment (such as competency recordings, case reports, case presentations, etc.)
- any piece of work that contains potentially identifiable third-party data or details (even if anonymised)
Consent requirements for your programme will be specified in your Programme Handbook which can be found on your ELE page. If you have read the guidance and are still unsure what you need to submit, please contact a member of your Programme Team.
Missing or incorrectly completed consent submissions
Your submission will be refused and deemed incomplete and a non-submission if a consent form (or recorded consent statement, where required) is:
- missing at the time of submission
- incorrectly completed (e.g. not fully signed by the third party, such as a service user)
Late penalties will apply to the whole assignment where consent is incorrect, incomplete or corrupted. Please see TQA section 11.4 for information on incomplete submission and late penalties.
General guidance on submitting consent for assignments
Allow yourself plenty of time to get the required consent for your clinical assignment.
When submitting consent forms, you must:
- use the relevant University of Exeter consent form(s) found on your ELE pages
- not submit any local or service consent forms. These will not be accepted as an alternative to a University consent form.
- ensure all required sections of the consent form are signed and dated
- include handwritten or electronic signatures. If you receive a typed signature, a consent email from the individual must be submitted alongside the university consent form (guidance can be found on your ELE pages).
